Community Health & Wellness Partners is driven by a mission to serve as community partners in healthcare. We believe in providing economic-based, whole-person, whole-community integrated primary care to citizens of Logan and surrounding counties to improve their long-term health and wellness. Community Health & Wellness Partners provides access to high quality care that improves the health and well-being of our community by providing medical care in Bellefontaine, Indian Lake and West Liberty. Our goal is to touch and serve all residents — those in need of general primary care; those who are managing diseases, fighting addiction, depression, or dealing with family matters; and those who are members of minority communities. Community Health & Wellness Partners offers medical care services to patients of all ages — from birth to death. Additional services include Pharmacy, Chronic Care Management, Integrated Behavioral Health, Medication Assisted Treatment, Wellness and Preventative Care.
